Employee Change Forms have been developed as a replacement of the Notification of Change Form.
The Notification of Change Form will no longer be accepted from 20th February 2023.
The form is tailored to the manager and their email address.
Managers who are based in Schools and Family Learning Centres such as Head Teachers, Depute Head Teachers, Head of Centres will be on a different domain (email) than staff who work for non-educational establishments such as Senior Social Workers, Education Managers or Service Managers.
Remember! You must be an Authorised Signatory to submit these forms!
